  1. Letter of Representation (LOR) — from attorney
  2. Photo ID
  3. Second ID (SS card, passport, work ID, etc.)
  4. Police Report
  5. Insurance Declaration Page (Dec Page)
  6. Health Insurance Card (if applicable & billable)
  7. Proof of Address — ONLY if not named insured
    • Cellphone / water / cable bill
    • Bank statement
    • Must include Date of Accident
    • Hospital bills not accepted
